Examples

These before/after examples show the three-section structure of WritingPAD output. The “after” panels display the combined output as you would paste it into your PAD or ePAD. WritingPAD also displays each section individually. All outputs were generated by the tool — not written by hand.

Examples are illustrative; always use your professional judgement before including feedback in a PAD. Note that the “before” examples include typical rushed notes with abbreviations, misspellings, and clinical shorthand.

Example 1: End-of-placement feedback — spoken via voice input

This example was spoken aloud using WritingPAD's voice input feature. The “before” text below is the raw transcription — no typing, no editing.

Right. Um. End of placement feedback for Jess, year two, medical ward, four-week placement. So. Overall she's actually been good but like needs to speed up. She's really nice with patients, very kind, probably too soft sometimes, like she hesitates before speaking up. She can do obs fine, NEWS scoring mostly correct, just needs reminding sometimes about documenting straight away instead of saying she'll do it later. She did forget once on Tuesday but we caught it before handover so no harm done. Medication rounds she's safe but slow. Triple checks everything which is good obviously but it makes the round drag. I'd rather that than unsafe though. She still needs prompting with insulin calculations, especially sliding scale. Communication wise she's polite with the MDT but doesn't always contribute unless asked. I've said she needs to be more confident because she knows more than she thinks. Time management is her biggest issue. She'll spend ages chatting to one patient and then runs behind. Needs to prioritise better when ward is busy. Professionalism fine. Uniform always correct. No sickness. Turned up early most days. Documentation handwriting okay, just sometimes too brief, like she assumes we know what she means. Needs to be clearer and more objective. No major concerns. Just needs to build confidence and work on pace. I think she's met the proficiencies for this stage but still developing obviously. Action plan confidence, prioritisation, faster documentation, more independent med calcs. Yeah. That's it.

Professional placement feedback (after)

The student nurse demonstrates a person-centred approach to patient care, maintaining respectful and considerate interactions throughout the placement. Physiological observations are performed competently, and NEWS scores are calculated correctly in most instances. Medication administration is conducted safely, with appropriate verification checks prior to administration. The student nurse maintains a professional appearance, with uniform consistently meeting the required standard. Attendance is reliable, with the student nurse generally arriving ahead of scheduled shift times. The student nurse communicates politely with the multidisciplinary team. Documentation is legible and proficiencies for this stage of the programme have been met. Contemporaneous documentation of physiological observations requires development; recording should occur promptly following completion rather than being deferred. The pace of medication administration rounds would benefit from continued practice to develop efficiency alongside safe checking procedures. Insulin calculations, particularly in relation to sliding scale regimens, require further development and are currently completed with prompting from the practice assessor or supervisor. Active contribution to multidisciplinary team discussions without prompting requires development. Time management during periods of ward activity requires attention, particularly in relation to prioritising clinical tasks when workload demands increase. Clinical documentation would benefit from greater objectivity and specificity, as entries are at times insufficiently detailed to be clearly understood without prior contextual knowledge. An action plan has been agreed to support the student nurse's ongoing development in the following areas: building confidence in proactive clinical communication and independent decision-making; developing prioritisation skills during periods of high ward activity; improving the timeliness and specificity of clinical documentation; and progressing towards independent insulin calculations, including sliding scale regimens, with reduced reliance on prompting.

Example 2: Observations and wound care — positive with development area

Professional placement feedback (after)

The student nurse demonstrates competent and person-centred patient interaction. Physiological observations are recorded accurately and in a timely manner, and NEWS scores are calculated correctly. The student nurse delivers basic wound care safely and effectively. Aseptic technique requires further development, particularly in relation to complex wound management, to ensure safe and effective practice. Further supervised practice in aseptic technique for complex wound care is recommended prior to sign-off of this proficiency.

Example 3: Disengaged student — emotional input sanitised

Example 4: Strong medication practice — all positive

Example 5: Care planning concerns — university contacted
